Short-Term & Post-Acute Rehabilitation

Burlington Convalescent Hospital provides post-acute and short-term rehabilitation services for individuals requiring clinical support following illness, injury, or hospitalization. Rehabilitation services are provided as part of a coordinated care plan and delivered by licensed professionals within the facility.

Comprehensive In-House Therapy Services

Rehabilitation services at Burlington Convalescent Hospital are provided on-site and may include:

Physical Therapy (PT)

Physical therapy services focus on mobility, strength, balance, and functional movement, as determined by an individualized care plan.

Occupational Therapy (OT)

Occupational therapy services address daily living activities and functional skills based on individual needs and clinical assessments.

Speech Therapy (ST)

Speech therapy services support communication, cognition, and swallowing needs when clinically indicated.

Therapy services are provided according to physician orders and individualized treatment plans.

Skilled Nursing Support During Rehabilitation

Rehabilitation services are supported by 24-hour skilled nursing care, which may include:

Medication administration and IV therapy

Wound care and skin integrity management

Ongoing clinical monitoring and coordination of care

Nursing and therapy teams work collaboratively within the facility to support residents throughout their rehabilitation stay.
